The problem
Maria Ivanova arrives for a crown prep. You open three places to find her story: a paper folder with an outdated odontogram, an Excel sheet with invoice notes, and a WhatsApp message from the previous dentist about a failed root canal on tooth 46. Her EGN is written wrong on the intake form. The NHIS number is missing, so you cannot verify НЗОК coverage. Last year's X-ray sits in a Gmail attachment nobody tagged. You spend nine minutes reconstructing context before you pick up a mirror, and you are still not sure the allergy note from 2023 made it across. This is what patient care looks like when records are fragmented. Every missing field is a compliance risk. Every untagged document is a clinical guess.
How it works in Udentis
Full electronic record with Bulgarian identity fields
Udentis stores every patient with the fields Bulgarian clinics actually use: EGN (ЕГН), three-name format (first_name, middle_name, last_name), NHIS number (НЗОК patient identifier), date of birth, phone, email, and full EKATTE address broken into region, municipality, city, and street. Search by any combination, type "Ivanova" or paste an EGN, and the record opens in under 300 milliseconds.
Medical history captures allergies, chronic conditions, current medications, and pregnancy status with structured checkboxes plus free-text notes. Emergency contact and preferred_language (bg/en) sit on the same screen. Reception completes intake in four fields and two clicks; the dentist sees everything without asking again.
Duplicate detection runs on EGN and phone at save time. If Maria Ivanova already exists, Udentis surfaces the match before you create a second record. Clinics report cutting intake errors by 70% and saving roughly six minutes per new patient registration.

Interactive FDI odontogram
The odontogram follows FDI World Dental Federation notation: teeth 11–48 on a clickable chart. Click tooth 36 to mark status, healthy, caries, filled, crown, missing, implant, root canal, and add surface detail (mesial, occlusal, distal, buccal, lingual). Colour coding updates instantly and persists to patient.tooth_map as structured JSON.
Planned treatments link directly from the odontogram: mark tooth 46 as "crown planned" and Udentis suggests scheduling a crown-prep appointment with the correct treatment kit. After each visit, completed work updates the chart automatically when the linked appointment is closed, no redrawing on paper.
Dentists switching from paper charts report treatment planning conversations with patients are 40% faster because the visual chart is on screen during the exam. "This is the tooth we treated in March" takes one click, not a folder search.

НЗИС dental history and chart import
For patients with a НЗИС identifier, Udentis can fetch dental treatment history from the National Health Information System using a live D009 request signed with the dentist's KEP via Udentis Desktop.
Fetched procedures appear in the patient timeline and can be imported into the odontogram. Results are cached so the history remains available between syncs without repeating the signing step every time.

Documents, photos, and radiograph archive
Attach any file to a patient record: intraoral photos, panoramic X-rays, CBCT slices, referral letters, lab work orders, insurance approvals. Drag-and-drop upload supports JPEG, PNG, PDF, and DICOM. Each file stores uploaded_at, uploaded_by, document_type, and optional tooth_number for filtering.
The gallery view groups images by date and type. Click tooth 36 in the odontogram and the sidebar shows every radiograph and photo tagged to that tooth, before-and-after comparisons side by side. No more scrolling through untagged email attachments.
Storage is encrypted at rest (AES-256) with per-clinic isolation. A typical crown case with six photos, two X-rays, and one lab PDF uses under 15 MB, well within standard plan limits. Retrieval is two clicks from the patient header, not a USB drive in the drawer.

Informed consent templates and GDPR compliance
Udentis ships with editable consent templates for common procedures: general treatment, surgical extraction, implant placement, sedation, and data processing under GDPR Article 6(1)(b). Merge fields auto-fill patient name, EGN, procedure description, dentist name, and date. Print or send for e-signature; signed PDFs attach to patient.documents with consent_signed_at timestamp.
GDPR tooling is built in, not bolted on. Role-based access ensures reception sees contact fields, dentists see clinical data, and admins see audit logs, nothing more. Data export (right of access) generates a ZIP with profile, odontogram, appointments, invoices, and documents in under thirty seconds. Deletion requests trigger a soft-delete with thirty-day retention before permanent purge, per Bulgarian health data guidelines.
Every view and edit on a patient record is logged: user_id, action, timestamp, IP. When the regulator asks who accessed Maria Ivanova's file on 15 March, you pull the audit trail in two clicks, not a week of IT investigation.

Who it is for
Solo practice
You are dentist and admin. Udentis gives you one patient screen with odontogram, history, and documents, no switching apps. D009 import means new patients arrive with НЗОК history already loaded. Consent templates print in ten seconds. You spend time treating, not filing.
Multi-dentist clinic
Four dentists share one patient database. Each provider sees the same FDI chart updated in real time after every visit. Reception handles intake with structured EGN and EKATTE fields; dentists open the record and start examining. Document gallery means any provider picks up Maria Ivanova's case mid-treatment without asking colleagues.
Clinic group
Patients move between locations. Maria Ivanova books in Sofia and treats in Plovdiv, one record, one odontogram, one document archive. Group admins run GDPR export and audit reports across all sites. NZIS D009 import works regardless of which clinic registered the patient.
